Instructions
When ordering, please specify the exact sort required: (1) membership category; (2) Country or zip select, (3) Fields of specialization -- primary and/or secondary plus the field number(s).
For a list of Fields of Specialization call 615-322-2595 and the information will be faxed to you.
We also require the following information: (1) type of labels, (2) keycode if desired (up to 6 characters), (3) date required by, (4) email address or shipping address, (5) billing address, if different than contact.
Unauthorized Use of List
All lists are for one-time rental and one-time use only. Unauthorized re-use will be billed at the regular rate.
